Approved Disease-Management Drugs

Approved Disease-Management drugs are covered drugs and supplies designated by the Plan that treat hypertension (high blood pressure), hyperlipidemia (high cholesterol), diabetes, and tobacco cessation. To encourage participants with these chronic conditions to obtain treatment, the Plan covers these drugs when medically necessary at the Approved Disease-Management Drugs benefit level shown in "Highlights of the Plan."

The drugs currently designated as Approved Disease-Management drugs are listed as Tier 1 on the Coverage Tiers (Formulary). Some drugs listed may require you to pay coinsurance at higher dosages. The list of Approved Disease-Management drugs is subject to change at any time.
